On behalf of the Genesee Valley Health Partnership, I am asking your support of a community-wide planning effort to promote and improve health.

The Livingston County healthcare community, through the leadership of the Livingston County Department of Health and with the support of the GVHP membership, is leading this two-year effort to understand and assess the health and well being of our community.

We are beginning this process with visioning the health of Livingston County. The GVHP would like everyone who lives, works, and learns in Livingston County to have a say in shaping a healthier community.

We would like to hear your opinion and would greatly appreciate it if you would complete a survey about the health and well being of our community.

This survey is one step in a two-year process designed to understand and assess the health and well being of our community.

Through the process, the Livingston County healthcare community, through the leadership of the Livingston County Department of Health and with the support of the GVHP membership, hopes to strategically identify, analyze and prioritize health needs in the county, while identifying ways and resources to address those needs.

The GVHP is a group of health, education, planning, and human service professionals who care very much about providing opportunities to make Livingston County the healthiest County in New York State.
